If you had a taxable event (distributions, conversions, loan defaults/offsets and rollovers out) from your NYSDCP account, your 1099-R tax form has been mailed. Your 1099-R will come in an envelope from the Bank of New York Mellon. If you have not received this, you can request a reprint by contacting the HELPLINE.
